Yes, the 3 keepers count as your first 3 draft picks essentially. You can trade prior to the draft. Say for example you had two awesome keepers, but no clear cut third one... you could trade one of your keepers for 2 other players that you'd keep.

But would the transaction take place after the draft? Say I want to keep Thome and Bedard. Now I don't give a fuck about the other players so I'll trade you my 3rd keeper spot (essentially my 3rd round pick) for two of your keepers? Or two of your players after you team is drafted? I don't know why this is confusing me so much.

No, it doesn't quite work that way. The keeper trades need to happen before the draft. It won't allow you to trade a keeper pick, but you can trade the players.
