From what I've read, he is implanted with an artificial heart (how his body didn't explode into 5000 pieces from the fall is beyond me), and needs to constantly jolt it in order to keep it ticking. Also, he goes on a wild goose chase when his real heart is stolen by ruthless Chinese mobsters.
This all sounds plausible to me.

I was thinking the first one could have been a dream and part 2 starts with the guy waking up, bathed in sweat.
